It's not an official form, but you can get an emailed PDF with an annual royalty payment breakdown if you go to your Annual Royalties page here:
Typically, authors who need something like this for their accountants use their Wise or PayPal records, since those are records of the actual payments received, but we hope this form can help in some cases.
Authors can also use the Royalty Payouts displayed here:

Emailing the Form to Yourself
When you go to your Annual Royalties page, it will look like this:
Click the "Email Me My Annual Royalties Statement" button to email the form to your Leanpub account email address:
The Royalties Payments Form Email and the Attached PDF
The email you will get with the attached form will look something like this:
The attached PDF will be named something like Leanpub_Annual_Royalties_Statement.pdf and will look something like this:
