When I add a new version of a Leanpub book to Kindle, it shows up as a duplicate. What can I do to fix this?

When you get a new version of a Leanpub book, you get a completely new ebook file.

If you are using an e-reader device or app, such as Amazon Kindle, when you get a new version of a book, this means you may see what appear to be duplicate copies of the same book in your e-reader or app.

Typically e-reader devices and apps make it easy to remove items you no longer want to see.

For example, if you press on a book cover image in the Kindle app on an iPhone, you'll see these options:
